  • Account Application Form
    		•  Download Form
      * Fill in minor's information as the main account holder and custodian's as the secondary account holder
      * Fill in minor's SSN as tax ID and minor's date of birth.
      * Only custodian(s) need to sign the application.
  • Foreign Custodian Account: A Copy of Minor's Legal Document
        such as Birth Certificate or School Record

  • Domestic Custodian Account: A Copy of Certificate of
        Appointment Issued by Court.

        Driver's License or Passport

    Domestic Account Signature Card for Checkbook
    If you want to have check-writing privileges, please complete and sign the Signature Card. Checkbook is only available for non-day trade domestic accounts.

    Account Transfer Form
    If you need to transfer securities and/or funds from another institution, you need to submit this form to complete your application process. Please include a separate transfer form for each account you're transferring along with a copy of the most recent statements for each account.

    Limited Trading Authorization Form
    To authorize agents to place trades for you, you need to fill out this form. Please be aware that this form only authorizes buy/sell transactions. Authorized agents for foreign accounts are required to provide photo identification.

    W-8 Form
    Each custodian must complete and sign a W-8 form to be exempt from U.S. taxes.

    W8 Account Proof of Address
    A foreign account using a U.S. mailing address will need to fill out this form to provide proof of the residence outside the U.S.

    Each custodian and their authorized agents need to attach a copy of each of their passport including name, photo and signature pages.
